A case manager is a nurse or social worker who will work with the family to help them to find programs and services to assist with their child’s special needs. Makes referrals to and/or assist families to access state, federal and community resources which may include, but are not limited to: NJ FamilyCare, Early Intervention Services, Social Security Income (SSI), Local charitable organizations, NJ Children’s System of Care.
Any child living in New Jersey from birth until their 22nd birthday with special health care needs or at risk for developmental delay. Legal citizenship is not required. Special health care needs include, but are not limited to: Long-term medical conditions, Autism, Physical disabilities, Birth defects, Developmental disabilities, Traumatic brain injuries.
